Description
International Affiliate Membership may be granted, upon approval by the officers, to those persons who have made written application who do not meet the requirements of physician membership, but who are licensed physicians in a country outside of the USA. International Affiliate Members shall pay dues equal to that of Physician members and shall meet the criteria and approval of the officers. International Affiliate Members may attend an educational meeting of the FAO upon prior approval of the FAO and/or the applicable course director, which may include a course prerequisite and verification of credentials.
Documentation Required (upload during checkout process): completed membership application; letter to FAO specifying type of practice and licensure outside of the USA, copy of professional license, CV.
