Important: Please check on the “Membership Categories and Eligibility” before applying for membership. Each category has specific eligibility requirements. You can also click the tile picture for a description of membership categories and eligibility.

If you are required to submit documents for your membership type as specified (such as verification of licensure, CV, or letter), you will be able to do so during the checkout process. During checkout at the top of the screen you will see a “upload file” button – please upload required documents as one PDF file.
